Caring For Someone With Dementia
At Daughterhood, we know so many members of our community are caring for someone living with dementia. Our partnership with PocketRN is designed to help make it easier for you. Through PocketRN, we are bringing you the GUIDE model – a pilot Medicare innovation that’s totally free and will provide you with a care coordinator, a 24/7 nurse in your pocket, respite and other support.

What is GUIDE?
PocketRN
PocketRN provides no-cost virtual access to an expert nurse—your dedicated Nurse for Life—to support qualifying families and their loved ones living with dementia anytime, anywhere.

The GUIDE Model focuses on comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities.
